Our mission is to uplift and advocate for youth through a variety of therapeutic programs and services. Our qualified therapists lead group sessions that focus on emotional well-being, while our Medicaid Targeted Case Management connects individuals with necessary resources.

Our Home Again Program supports elderly citizens transitioning back to their communities from healthcare facilities. At COHYA, we are committed to fostering hope and resilience in every individual we serve.

Youth Therapeutic Support Groups:
Ages 7-11  and  12-16


In our "Parents Just Don't Understand" Support Groups, we provide essential guidance for youth facing the challenges of adolescence. 

Our trained therapists facilitate discussions around relevant issues such as peer pressure, effective study habits, and healthy coping mechanisms.

Medicaid Targeted 
Case Management Services

COHYA gives individuals—especially women and families—a voice, a plan, and a pathway forward.

By bridging the gap between healthcare, social support, and community services, COHYA empowers people to navigate challenges with confidence and care.

Home Again Program


With COHYA, you are not alone in your transition. 


Our Home Again Services, in collaboration with SCDHHS, provide essential support to Medicaid beneficiaries who have resided in nursing facilities or hospitals for over 60 days who are prepared to return to the community.



We provide the guidance, resources, and encouragement needed to help you successfully transiton home.
