Our programs are designed to help youth, families, and individuals regain stability, independence, and hope through holistic advocacy and support.

We partner with community organizations and agencies, including the South Carolina Department of Health and Human Services (SCDHHS), to ensure that every person we serve receives comprehensive, person-centered assistance that leads to lasting change

  • "PARENTS JUST DON'T UNDERSTAND"   Youth Therapeutic Support Groups:   Ages 7-11  and  12-16

    In our "Parents Just Don't Understand" Support Groups, we provide essential guidance for youth facing the challenges of adolescence. Our trained therapists facilitate discussions around relevant issues such as peer pressure, effective study habits, and healthy coping mechanisms. Together, we can equip young people with the skills to overcome obstacles and aspire to honorable futures.

    🩺 Medicaid Targeted Case Management (TCM)
    Personalized care coordination and advocacy connecting youth and families to vital healthcare, counseling, housing, and social resources. Our Case Managers ensure each individual receives consistent follow-up, guidance, and empowerment toward stability and self-sufficiency.

    COHYA provides Individual Therapeutic Services to address the emotional and behavioral health needs of youth and families.
    Through compassionate counseling and evidence-based therapy, we help clients overcome trauma, anxiety, depression, and life challenges—supporting long-term emotional growth and well-being.

    šŸ  Home Again Services (In Collaboration with SCDHHS)
    Through the Home Again Program, COHYA assists Medicaid beneficiaries who have resided in nursing facilities or hospitals for more than 60 days.
    Our team helps individuals safely transition back into the community by coordinating housing, healthcare, and social support—ensuring a smooth return to independent living with dignity and stability.
